Frequently Asked Questions
How do I make an appointment?
We currently do not make appointments in advance. We see patients on a first-come, first-serve basis on clinic nights. Registration begins at 4:30pm and ends at 5:30pm.
Can you see my child?
Yes! We have a pediatrics clinic on the first Wednesday of each month, from 6:00pm-9:00pm
What happens if I require additional tests or medications?
We do our best to provide as much on-site care as possible, but sometimes we just aren’t equipped to handle all situations. If you require additional testing not offered at our clinic, we do our best to get your treatment covered at the Good Samaritan Lab or provide you with a UK Financial Assistance form. For medications not stocked in our pharmacy or available on a $4 list, we provide referrals to Faith Pharmacy and/or help you complete a Prescription Assistance application.
Are you able to provide a determination of disability?
No, our physicians do not provide disability determinations.
I was seen at your clinic in the past and now require my medical records to be sent to my current doctor. How do I go about this?
We are happy to provide your medical records to your current provider for continuity-of-care purposes. Please mail or fax us your request with the appropriate signed medical records release form, and we will send your records as soon as possible.
